For participants under the age of 18, parental or legal guardian consent is required to complete the registration process. The registration fee is a one-time payment, non-refundable, and necessary to secure your child’s spot in the academy. Tuition fees for memberships, after-school programs, or additional services must be paid according to the schedule and payment methods provided by JP Smiles. Failure to make timely payments may result in suspension from the program until the account is resolved. Program schedules, locations, or content may be adjusted by JP Smiles if necessary to ensure the quality and safety of the activities. Families will be notified promptly of any changes. Attendance and punctuality are essential for your child’s progress and participation in the program. Parents are responsible for ensuring their child arrives and departs on time. JP Smiles is committed to the safety of all participants. Parents or guardians must inform us of any medical conditions, allergies, or special needs their child may have. In case of emergency, JP Smiles staff will contact the parent or guardian immediately and, if required, seek professional medical assistance. We take privacy seriously. All personal data provided during registration, such as contact details and medical information, is collected solely for administrative and safety purposes. This information will not be shared with third parties unless required by law or with explicit consent. JP Smiles uses photographs and videos taken during activities for promotional purposes. Parents who do not wish for their child’s image to be used must notify us in writing at the time of registration. By enrolling in JP Smiles Academy, parents and guardians acknowledge their understanding and acceptance of these terms and conditions. PHOTO/MEDIA CONTENT
I hereby grant permission to JP Smiles to use my image on its website, Instagram, or Facebook page without further consideration. Additionally, I grant permission to use images of my children listed below. I understand that no names will be used on the website unless specific written permission is given to an appropriate staff member. I acknowledge that once any image is posted on JP Smiles' website, Instagram, or Facebook, it can be downloaded by any computer user around the world. I hereby attest that I am the legal parent/guardian of the child(ren) listed below. This consent is effective until such time as I revoke it in writing and provide a copy of the revocation to JP Smiles.
